On September 25-26, 2015, the UCSF Center for Excellence in Primary Care (CEPC) hosted the Tom Bodenheimer Symposium and Gala in San Francisco. This event celebrated Dr. Thomas Bodenheimer's transformational work and numerous contributions to primary care.

Attended by industry leaders and innovators, the Symposium was designed for participants to take part in open conversation with their peers to learn, grow, and build networks.

Our team was part of the Symposium:  MacColl Emeritus Director Ed Wagner facilitated a breakout session entitled "Rethinking the Way We Do Business:  Lessons and Evidence from the Practice Transformation Field", and Associate Director Brian Austin led Innovation Cafe sessions on improving primary care using our LEAP Team Guide.  He also highlighted our work with the Safety Net Medical Home Initiative and Coach Medical Home.

CEPC describes Tom Bodenheimer's spirit in this way:  "thinking creatively, embracing innovation, learning with humility, advocating for what is just, not taking yourself too seriously, and enjoying the journey".  We are so proud to be associated with Tom as colleagues, collaborators, and friends.
