This January 2015 paper, co-authored by Michael Parchman, outlines seven trends in providing sustainable primary care practices support. It provides an overview of the evidence on effectiveness of external practice support in improving process and health outcomes in primary care. "Emerging Primary Care Trends and Implications for Practice Support Programs" feature three case studies of high-performing practice support programs, with a focus on how these programs have sustained funding and infrastructure in a changing environment. Finally, it presents recommendations to practice support program leaders for maintaining financially and strategically viable programs as the U.S. health care system, and specifically primary care practices, move through a time of significant change.
