The MacColl Center for Health Care Innovation website and Implementing Innovations into Practice collaboration platform were designed to help delivery systems improve their care for individuals with chronic conditions through the dissemination of information to clinicians, researchers, and others interested in healthcare quality improvement.

We collect no personally identifiable information about you when you visit our Web sites unless you provide us with this information. We do not share personal information with third parties except as would be required in the event of a legal process.

We collect standard server log information (i.e., domain name or IP address from which you access the Internet, the date and time you access our site, the pages you viewed). We use anonymous browser “cookies” to track file usage from all visitors.

All automatically collected information is collected on an aggregate basis only. Our Web site logs are not personally identifiable and we make no attempt to link them with the individuals who browse our site. For example, we track which pages are most popular among visitors as a whole, but pages viewed are not identified with individual users. With email inquiries, we store the inquiry and the email address information so that we can respond electronically.
